1) Income & Employment
Salaried:
  • Complete company name, mailing address and telephone number for employers covering the last twenty-four months
  • W-2’s for all employers covering the last two years
Self-employed:
  • Federal IRS forms 1040 (personal returns) for the last two tax years, including all addenda
  • Federal Income Tax filing forms for business, including all addenda
2) Assets
  • Provide the most recent complete statement for all asset account (including all pages)
  • If a gift is to be received provide the donor’s full name, address, and telephone number
3) Fully executed Purchase and Sale Agreement
